Effective February 10th, 2008, Protective Marketing Enterprise, Inc., is the operator of the USA HealthCare Savings medical discount programs, with its corporate offices located at 4929 W. Royal Lane, Irving, TX 75063. The Capella Group, Inc. will continue to be the program operator in the state of California, Florida, Indiana, Oklahoma, Tennessee and Utah.

Important Information About the Program:

The USA Healthcare Savings programs are NOT INSURANCE but provide discounts at certain health care providers for medical services.  The discount program does not make payments directly to the providers of medical services.  The member must pay for all health care services but will receive a discount from those health care providers who have contracted with the program operator, Protective Marketing Enterprises, Inc.  The range of discounts for health care services provided through the program will vary depending on the type of provider and service received.  Equal or lower pricing may be available through individual negotiations.  Prescription discounts are available only at participating pharmacies.  This is not a Medicare prescription drug plan.  An up-to-date list of providers, including the provider’s name, city, and specialty is available before purchase and upon request. Royal Ln., Ste. 200, Irving TX 75063 and can be reached toll-free at (888) 411-3888 or at www.usahealthcaresavings.com.  You have the right to cancel your membership at any time, and if you cancel your membership during the first 30 days by notifying us in writing and returning your membership cards, you will receive a full refund of membership dues within 30 days.  Nominal enrollment fees are non-refundable except as required by law.  In Florida the term “program” means “plan.”

Refund Policy:

Members who cancel during the first 30 days of membership by notifying USA Healthcare Savings in writing and returning their ID cards will receive, within 30 days, a full refund of membership dues. Any application/processing fee is not refundable (except in states where prohibited). Members may cancel after the first 30 days, and, in states where required, will receive a pro-rata reimbursement of all periodic charges.
